About Guy`s and StThomas Home from Hospital Service

To use this service you need to be 18 or over, living in Southwark &Lambeth and recently discharged from the hospital. This service does not provide personal care or medication and we can`t help move or liftyou. Should you require a higher level of care, the service will be able to signpost you to a relevant agency or community support service.

The service is free to use. It is currently funded by the Guy’s and St Thomas’ NHS Foundation Trust.

Our service aims to help you recover and prevent you going back intohospital. We will do this by settling you back home, helping you toaccess longer-term support if needed and by working with appropriatemedical and social care staff.
