Digital ICU Recovery Pathway

What is the digital ICU recovery pathway? 

The digital ICU recovery pathway provides support to you and your family during your recovery from ICU. This support can start once you leave ICU and remain in place until you are seen in the critical care recovery clinic at 2-3 months after returning home. The pathway is centred around setting and achieving your recovery goals and is delivered via an app on your phone/smart device. You are supported by a dedicated recovery coordinator throughout your recovery journey.

 

What are the criteria to be considered for the pathway? 

  • Only patients from Guy’s and St-Thomas’ NHS Foundation Trust are eligible to be recruited 
  • Supported by a breathing machine (ventilator) for five days or more during your ICU stay 
  • Being medically stable
  • Able to use a smart device with or without help
  • English speaking or having a family member that can assist you using the app

 

What is the Recovery coordinator role? 

The ICU recovery coordinator supports you during your recovery by:

  • Helping you to set recovery goals
  • Making referrals to other healthcare professionals (such as doctors and dieticians)
  • Directing you to other community services (such as psychological support)
  • Providing occupational therapist specialist care and advice including information on return to work, financial aid, and fatigue management.

 

How can I show my interest for the pathway if I would like to be enrolled? 

If you are interested in being enrolled on the pathway and getting more help during your recovery journey, you can tell your treating team so that they can reach out to the pathway coordinator.
